Our confidential, personalized, multipage report provides a current health status and individual risk factors based on their integrated laboratory and health questionnaire data. It offers detailed action plans with gender and age specific content, plus preventive screening and dietary information. A dynamic online report also explains and shows the body systems and provides definitions for risk levels.
Blueprint for Wellness My5 to Health Profile™
My 5 Health Profile™ is a robust report that makes your screening dollars work harder. Metabolic Syndrome is medically sound, easy to understand and its five risk factors – Waist Circumference, Body Mass Index, Triglycerides, Glucose, HDL Cholesterol and Blood Pressure - ideally suit employers considering outcomes-based incentives. Participants are given a simple score reflecting how many Metabolic Syndrome target values they passed. Participants who achieve 0-2 target values are 5 times more likely to become a diabetic, and 2 to 3 times more likely to have a cardiovascular event. Participants who achieve 3-5 target values are at lower risk.

MyTest Profile™ is a 2-3 page report which provides employees with information on their laboratory results, expected ranges and a brief description of the tests performed. This communication gives your employees the opportunity to "learn and become familiar with their numbers." This information provides a baseline from which the employees can work to improve their overall health and reduce their health risks.

OurCompany Profile answers the question, “What is the health of our company?” This comprehensive, aggregate report summarizes the results of your employee population — yet maintains employees’ confidentiality with no personally identifiable information. Return-on-investment data provides a meaningful financial forecast of excess healthcare costs due to modifiable health risk factors so employers can create effective intervention programs and measure the effectiveness of existing programs. The integrated data represented in our health profiles provides actionable information back to both the employee and employer — insightful information of a magnitude that is not typical of programs based solely on self-reported health information.
