- Your organization communicates information directly to you about your registration key and Unique ID. If you have questions, please contact your organization for your specific information.
Unable to attend an event
- If your progam offers an off-site screening option, you can visit a Quest Diagnostics Patient Service Center for a blood draw instead of attending an on-site event. Once you register you will be given instructions on how to receive an off-site information kit in mail. The kit will contain instructions on locating a collection site near you. You can not go to a Patient Service Center for a screening without the kit you will recieve in the mail.
Find a collection site
- You will need to choose "Doctor's Order" then "Routine" as a reason for testing.
Take your health questionnaire
- You will be required to enter your registration key and Unique ID to complete your health questionnaire.
- Your organization communicates information directly to you about your registration key and Unique ID.
- If you have questions please contact your organization for your specific information.
View results
- You can only view results if you completed your health questionnaire online.
- Please allow at least 5 days after your blood draw to try to view your online results.
- To view your results you will need to remember your initial log in information.
