A customized packet is sent to each employee introducing them to the Blueprint for Wellness program. | |

The employee will be instructed on how to complete the Health Questionnaire (if selected) and locate a Quest Diagnostics Patient Service Center (PSC) and schedule an appointment to have their blood drawn. |

Employees take their test order to the nearest Quest Diagnostics PSC for their blood draw. Quest Diagnostics has 2,000 PSCs located across the United States. | |

The employee will receive the results of their screening both online and via the mail. See sample reports with Health Questionnaire or without Health Questionnaire. | |
