What is a Health Questionnaire and why is it Important?
The Blueprint for Wellness Health Questionnaire is a comprehensive online questionnaire to provide data related to health, diagnosed conditions, lifestyle and related information.  When integrated laboratory results, a comprehensive report is produced that identifies  individual risk factors, current health status, and resources for improvement.

Are there fasting requirements in the Blueprint for Wellness Small Business Solution?  
Yes, both laboratory panels require a fast of 9-12 hours.  During this period, employees should not  eat or drink any items other than water.  As part of the employee communications, fasting is highlighted as a requirement for screening.
 
What happens if an employee has an alert value on the laboratory result?
Consistent will all Blueprint for Wellness programs, medical oversight is provided by Quest Diagnostics' contracted  physician group, Physician Wellness Network. Employees with alert level laboratory results will receive a phone call from one of our contracted physicians to inform them of the value and guide them to follow up with their personal physician.
 
Is there a deadline for participation in  the program?
Yes, all Blueprint for Wellness programs have a 60-day  window in which testing must be performed, this deadline for participation is provided in the employee communications.

What are Quest Diagnostics Patient Service Centers (PSC)?
Quest Diagnostics owns and operates over 2,000 Patient Service Centers across the country.  These locations, which offer service Monday through Friday and even Saturday in some locations, provide employees with a convenient option for their blood draw.  Employees can be serviced on a first come, first serve walk-in basis or make an appointment.  To find PSC locations, visit the below  link:
